To provide the best care for patients, it is essential to know as much as possible about their entire experience. This includes being aware of any acute symptoms present during and after treatment, as well as patients’ longer-term satisfaction with the results of their treatment and care. This knowledge enables the staff to improve care to benefit the next patient.

The MSK Patient-Reported Information for Strategic Management (PRISM) program seeks to support our clinicians and researchers in their mission to enhance the quality of care and advance research through a patient-centered approach. Our team members are leaders in the development and implementation of Patient-Reported Outcome Measures (PROMs) in clinical care and research and are driving efforts to incorporate PROMs into standard of care practices across MSK.

MSK PRISM platforms help our healthcare teams be more proactive as they follow patients from diagnosis through survivorship.  Our approach encompasses all aspects of care beginning at the first outpatient appointment and continuing throughout the patient’s recovery both at the hospital and in their homes after being discharged. All PROMs are delivered via MSK Engage on the MyMSK patient portal.

Key components of the MSK PRISM program include:

  • Improving patients’ experience and quality of life
  • Enhancing communication between patients and the clinical care team
  • Empowering patients to take an active role in their care

These goals are accomplished across our three PRISM platforms:

MSK PROS Oncology

MSK PROS Oncology follows patients from diagnosis to survivorship in the outpatient setting. At every visit, patients are asked to record Patient-Reported Outcome Measures regarding their function and the emotional aspects of their diagnosis, treatment and survivorship. These are reviewed by the treatment team and used to monitor short- and long-term outcomes across care as well as to provide early interventions when needed. As we place high value on the time and energy of each patient treated at MSK, our clinical experts define disease-specific timepoints for when patients are asked to record these measures, the first being assigned around the time of your initial visit with the doctor.

Recovery Tracker

The MyMSK Recovery Tracker is an electronic patient symptom self-reporting questionnaire sent to patients after their surgery. Alerts based on responses are generated and sent to the care team allowing for continuous communication after surgery.

MSK LYNC

As part of the Discharge Planning process, our surgical care teams outline the specific goals and milestones that postoperative patients must complete before leaving the hospital. MSK LYNC, Linking Your Needs with Care, is an electronic checklist updated by patients via the MyMSK Patient Portal. The e-Checklist communicates exactly what the goals are and empowers patients to track and update their individual progress on a daily basis. The system alerts the treating team about the patient’s progress toward a health status appropriate for discharge and enables early identification of patients who need extra help to assure they are safe and comfortable when they leave the hospital.
